世界的に平均寿命が延び、加齢や生活習慣に起因する非感染性疾患(NCD)の有病率が高まり、複雑な医療ニーズを抱える患者や高齢者の数が増加しています。 これらの患者や高齢者には、健康的な免疫機能をサポートし、医学的合併症を減らし、患者の回復や自立を促進する最適な栄養学のケアが欠かせません。 しかし、疾病関連の栄養失調、つまり疾病や疾病の治療に起因するエネルギー、プロテイン、そして微量栄養素の摂取不足により起こる状態は、多くの臨床症状で共通して見られ、予後不良につながる可能性があります。そのため、体が弱っている患者の栄養不足に対処し、可能な限り最良の臨床結果を得るために、経口栄養剤や経腸栄養剤などの医療栄養製品が必要とされる場合があります。
新たに発表された論文 では、エイコサペンタエン酸(EPA)およびドコサヘキサエン酸(DHA)の摂取と患者の健康を関連づける科学的根拠が蓄積され、これらの成分が高齢者や患者をどのようにサポートするかが検討されています1。実行可能な研究により、専門家委員会は、 EPAとDHAを含む経口栄養補助食品、または経腸・非経口処方の提供は、様々な治療領域で多くの患者集団をサポートするのに役立つと結論づけました。
ここでは、この論文で発表された重要な結論と科学的知見を紹介します。
非感染性疾患の患者は通常、慢性炎症を患っています。そのため、炎症の解消をサポートし、体内で適切に調節されるようにすることは、疾病と向き合う上で適切であると考えられています。長鎖オメガ3脂肪酸であるEPAとDHAは、人間の健康や正常な免疫機能に関与することが知られており、その主要な作用の1つに、炎症を抑えて解消を促進することがあります。1,2,3 この幅広い作用が、多くのさまざまな治療領域において医療合併症を減らし、患者の栄養ニーズをサポートすることが研究で示されています。4,5,6,7 今回の発表で、専門家委員会は、認知機能、加齢に伴う筋肉量の低下、がん、外科手術患者、重症患者など、多くの患者集団および病状においてEPAおよびDHA栄養の役割を特定しました。8,9,10 その結果、長鎖オメガ3脂肪酸を十分に供給することは、これらの病状を呈する患者の栄養管理における重要な要素と見なされるべきであるとされました。
ここでは希望の持てる結果をご紹介しましたが、現在利用できる研究は限られており、サプリメントの摂取量、タイミング、期間、ベースラインの栄養状態、臨床状態、薬の使用などのばらつきがあるため、一貫性がありません。例えば、がん性悪液質の治療には複合的な介入が最も有望であることが認識されつつありますが、臨床エビデンスの多くは単一の治療法のみを用いた試験から得られています。同様に、DHAおよびEPAが周術期の免疫栄養に役割を果たすことは明らかですが、よりよくデザインされた試験を行うことで、その使用に関する明確なエビデンスが得られ、最適なタイミングと期間が確認されると考えられます。これは、特定の患者群におけるEPAおよびDHAの効果をさらに明確にし、より個別化された効果的な医療用栄養製品の開発を促す、より良い対照介入研究の必要性を強調しています。さらに、医療栄養業界では、より食べやすく魅力的な医療栄養製品を製作するため、最新の患者の嗜好を教育するための継続的な患者インサイトが重要です。
